NHANES The National Health and Nutrition Examination Survey
NHANES contains freely-downloadable data on 10,000 patients including demographic, socioeconomic, dietary, and health-related questions. The examination component consists of medical, dental, and physiological measurements, as well as laboratory tests administered by highly trained medical personnel.

Timely and Effective Care measures - provider data. This data set includes provider-level data for measures of cataract surgery outcome, colonoscopy follow-up, heart attack care, emergency department care, preventive care, pregnancy and delivery care, and cancer care.
